If weight loss has made your face look older, rapid loss can reveal age-related changes in facial volume, muscle, structural support, and skin laxity. Board-certified dermatologist Dr. Shereene Idriss explains why “Ozempic face” is better understood as visible facial change after weight loss—not a medical diagnosis unique to Ozempic—and what may help support your face while your weight changes.
This guide covers facial fat loss, hollow under-eyes, sunken cheeks, jowls, loose skin, and the “sinker vs. sagger” framework. It also explains how GLP-1 medications such as semaglutide and tirzepatide affect appetite and weight, why weight loss can include lean mass as well as fat mass, and why protein, strength training, steady weight loss, skincare, and weight stability matter.
You’ll also learn how a dermatologist may think about skin tightening, conservative volume restoration, biostimulators such as Sculptra, fat transfer, and surgery after weight has stabilized.
This video is educational and does not replace individualized medical care. Prescription GLP-1 medications and cosmetic procedures require evaluation by an appropriately qualified healthcare professional. Do not change a prescription dose or treatment plan based on this video alone.

Your information has been the best I've seen. Thank you so much for all of the great advice. I'm a sinker and a sagger. Hopefuuly I can implement your regimen in my daily life.
Saw someone drop 8KG after reading The Forgotten Code of Permanent Weight Loss by L. Hale. The food cravings part sounds different.
do you still lose face fat if you're working out to put on weight instead?
I was hoping that when you started talking about tightening the face you were going to get into Sofwav, Xerf, microneedling, Everesse, Ultherapy… Which is best…Have you/will you? Please🙏❤
What about micro needling with exosomes

On another note Dr. Idriss I just got my hands on the Major Fade Disco Block SPF 50 and I love it 💗 it is so light weight and sat well with all my other skincare products!
Weight fluctuation over the years damaged the integrity of my skin, but I lost 95 lbs in my early 30s and had fairly little that I was unhappy about. Fast forward to my early 40s I got off birth control and my skin deflated. It came upon me – aging – so fast. Now I feel like I’m spinning plates trying to maintain what I never had to worry about. I always thought I’d lose weight in the future and go back to what I used to look like, and I always looked better with a thinner face. Now I’m legit afraid to lose weight because of my neck skin, inner arm and thigh skin, and stomach.
I learned about weight loss the hard way. Got pregnant with twins in my early 40s. I had hyperemesis gravidarum so I lost weight rapidly in my first trimester. Then put it all back on at the end of my second trimester. Then had to lose weight again after having my twins. I lost a lot of volume in my face. 12 years later I think I’m ready for surgery. Sigh….
Beautifu;l, radiant, articulate, I'm in love!
It’s spelled AGEING
Everyone is talking about me at work. I hear them whispering. I eat A LOT of protein and lift weights. My face is sinking AND sagging too!
She's the best. Thank you Dr Idriss xx
The first place i lose weight is on my face . And its so problematic. Thankyou for making this video
I really noticed it in my sister. The lines showed up in her normally plump face. Most of my family is overweight and for the most part they don’t have wrinkles. My theory is that a plump face hides the wrinkles.
I'm 72. Face is going to sag as i lose. Fact of life
Thank you Dr. Idriss for providing such a thoughtful and thorough explanation of what’s happening when we lose weight, what to focus on and options for in and out of the office. I appreciate this education so much.
Is it accelerating aging in the body that we cannot see?
Low and slow. Anything more dramatic is going to be more challenging to maintain. I think muscle stimulation works the best. Radio frequency combined with a (get ready for it) low dose stimulant along with collagen/protein powder. I know I’m going to get heat for it, but when I’m on a very low dose stimulant I lose a little bit of weight, and everyone tells me I look younger probably because my face looks tighter.
I’ve lost 165 lbs and no one thinks I’m 44. It’s so amazing lol
Im a sagger i guess. Had a round face its dropping lower plus volume loss
The first fouurteen minutes she doesnt say anything
EVERY SINGLE DAY
So late🥲 i have lost 25 kg
You first have to ask the question, why are you losing weight? Most women with normal BMI just want to lose waist, not weight but the older you get, the more your hormones work against you on this. The best strategy is to gain muscle through progression strength training especially in the lower body and reduce stress. Then if you have problem areas, treat with lipo. Even if you can't afford the lipo, you'll still look better and be healthier than ozempic.
Fairlife milk abuses their cows — avoid it!
My daughter in law is using the jab looks older now and doesn’t look healthy at all ,😔I’ve started using psyllium husk it seems to fill you up ,I guess it’s similar to the jab ..
how is your neck so nice and smooth ? well your whole face hah but specifically the neck – is there a video for that ? 🙂
Ok good to lose weight but fat and old looks better thats true so just a little
This is totally true. I’m in my 50s working out eating healthy to lose fat, and I am worried about how much older I will look because I have always had a fuller face even when I was in the best shape of my life. My concern is that the loss of fat will make my jowls or make me look old.
I am 53, not one wrinkle and began a microdose glp-1 in October. Lost 27 lbs. Trying to go slow so my skin doesnt sag. I will die if I lose my boobs or look mt age as I prided myself for great skin and losing wejght slowly to not have sagging skin.
I just saw a couple of photos of myself from a distance and at a weird angle, and I look about 80 and I'm 46. I just lost about 15 lbs and I thought I looked basically the same in person. Somehow the camera picked up a split second view of how much looser my skin really is.
I think HA kind of makes me look more like a sagger, and I wish they'd stop putting it in things. Maybe I'm just allergic, or it makes me more sensitive to things.
Absolutely love all of your videos. I dont get time to keep up with everyone I'm subscribed to as sometimes I am focused on improving my motorcyle skills, looking for protective clothing, other times I may be brushing up on my anatomy and physiology or painting and decoration but when it's skin I always come back to my favourites! Intelligent, well informed and funny.. Thank you Dr Idriss for your content ❤
too much intro zzzzzzzzzz. get on with it….
I picked both my face and my ass, and the results were very good for a person my age. No fillers, only Botox and under-eye fat transfer. I would be considered sagger, and my surgeon fixed that and tweaks my face and neck every 7 years. Yes, it's about maintenance, sunscreen, hydration, and sleep.
My body is all a result of strength training and lipo on those hard-to-lose areas. I was put on GLP because my weight was high, yet I was always a thick girl and felt comfortable in my own skin. I am currently participating in a study, and now my weight and fat are being managed monthly by the study, doctor and nutritionist. The medication is still NOT on the market. I was educated to prioritize protein, vitamins, and exercise using weights, to protect my muscles and bones. Yes, if you lose weight too fast, your face suffers the consequence, but in my case, I was provided the lowest dose of a new GLP, and losing one pound a week is encouraged with healthy eating and exercise. A GLP is not a magic potion; you must incorporate many things into your life to maintain fat in your face and keep your muscles.
a lot of people actually look younger after losing weight
