Want my exact peptide protocols?
Everyone’s chasing the 12mg retatrutide dose for maximum weight loss — but the trial data tells a different story. In this video, I break down the Phase 2 (NEJM 2023) and Phase 3 (TRANSCEND-T2D-1, Lancet 2026) data comparing side effects, dropout rates, and weight loss across every retatrutide dose. You’ll see why 4mg matched placebo on discontinuation rate, why the weight-loss curve flattens well before 12mg, and the pharmacological case for why 4mg may be the exact dose where retatrutide’s third receptor — glucagon — actually turns on.
If you’re on retatrutide, considering it, or trying to understand why dose selection matters beyond “more is better,” this one’s for you.

I put together all of my peptide protocols — the ones I actually use clinically with my patients. Weight loss, longevity, post-heart attack recovery, chronic disease, autoimmune conditions, all of it.
It's called The Peptide Blueprint: https://www.drkjoseph.com/offers/hZXAcxQ2/checkout
6 months in Reta, 4 mg is def the spot. I eventually titrated up to 7 for a month and noticed weird mood swings. Didn’t like it
Does the tingling going away eventually? Also does the heart rate come down once the body is adjusting to reta??
I want to start at the 4, but I need help reconstituting the 20mg bottle of peptides. How much bac water do I use
I have side effects at .25…I don’t know how y’all do it
I wonder how many of these "Dr's" on here talking about reta are actually real licensed PhDs when Reta itself is barely legal to obtain. You cant even talk about vendors on reddit but yet youtube is filled with all these "experts". Pretty sure these people just bought scrubs on amazon and then created a platform on youtube to call themes Dr's. I am a google engineer and I can verify for a fact that the youtube division does not verify the credentials of these people claiming to be doctors.
Can we split the does into 2mg twice a week or 4mg at once to see the benefits…
HOW MANY TIMES A WEEK DO YOU NEED TO TAKE THE INJECTION? I get the dose, but how many times a week please? can someone answer this question? Thank you everyone
I came to watch this video because I started reta a more ago and at 3 ml I have lost 3 kg with not side effects. Except that my stool is very solid and my emptying is very slow. But I like that I see the numbers change on the scale. I will bring it up to 4 ml after watching this
Where are people getting this product?
For beginniners you suggest 4mg
Im guessing I need to go up to 4 at least to start seeing results?? I was on 2mg for two weeks and lost maybe 2 lbs which was probably water? then I went to 3mg and maybe another pound in the past two weeks, and the past week nothing.
I'm eating plenty of protein
Water and electrolytes
Sleep
And strength training 3-4 times a week maintenance style.
Honesty somewhat disappointed but if I'm ONLY loosing fat then I guess it's not terrible, I see some loosing 2+ lbs a week 😮…. To be fair I think I can only loose about 10-12 more pounds and I just started on tesamorelin 7 days ago and I heard that makes you retain some water up front and the visceral fat takes over a month to start noticing anything??…. So maybe it's the Tesamorelin water retention. Just 1mg BTW.
But definitely going to 4mg this week🤞
When referring to 4mg can you micro dose 2mg twice a week?
always start slow with any drug starting at 1.5 is perfect. You don’t wanna go too high and then quit just our bodies aren’t designed to lose more than 8 pounds. It’s just not healthy. We don’t wanna lose her muscle. It’s better to lose one to 2 pounds a week and go very slow that way you have no side effects and get the weight off slow and steady win the race.
Always start at the lowest dose 0.5mg and if you need to go up do it slowly until you get to your desired level that’s what you need to do 😊
Is that 4mg per day?
I’m relatively lean and I have been on reta for about 5/6 weeks. 0.5mg and then went to 0.8mg worked amazing for me. Fat I’ve struggled to lose is finally going. I strength train 4/5 times a week and eat relatively clean. I was cutting a while back but I am now a bit more relaxed with my calorie counting but honestly it’s great. Make sure you are drinking electrolytes xx
what is the frequency? or did I miss that? Is it 4mg once a week?
How many units is this
The sweet spot thing matches what I ran into exactly. I titrated 2mg for four weeks, then 4mg, and sat at 4 for almost three months because appetite suppression was already more than I could out-eat. Went to 6 out of curiosity and all I really got was worse sleep, a resting heart rate about 8 beats higher, and reflux that had me sitting up at night. Scale moved maybe a pound faster over five weeks, then I dropped back to 4 and everything settled again. The dose that felt strongest was not the dose that worked best.
Two practical things nobody mentions. First, at 4mg the nausea was almost entirely food dependent for me, greasy dinners were miserable, plain protein and rice was fine, so half of what people blame on the dose is really the meal. Second, get your reconstitution math right before you touch the vial, I use bac water to a round number of units so 4mg is an easy mark on the syringe instead of squinting at a weird fraction. I get mine from Aureus Research since they publish the COA per batch and it ships cold from inside the US, which matters more than people think for something you are keeping in the fridge for two months. Also worth logging body weight and waist separately, my waist kept shrinking for weeks after the scale stalled at 4mg and I nearly chased a higher dose for no reason.
So 4mg dose a week or?
I am on one mg RT and 5mg TZ and in 13 weeks my visceral fat is down to 7 from 15! 40 lbs down, too but my liver is why I started the meds.
I'm on 4mg as well. I can attest to everything he's shared.
I lost the first 60lbs dieting on high protein, low carb. The next 35lbs I've lost on Retatrutide. I'm now at 200 lbs 6ft 3" 40 yr old male. Starting weight was 290 November 2025.
So do I start at 4 mg or should I start small and work my way up?
Evidence suggests that at 4mg, the glucagon receptor begins to activate
Data on Pancreatic response please – also do you stay on this forever ?
I've used for months, gain weight not lost😢
@drkjoseph1 are you referring to 4mg per week or 4mg twice a week?
Just moved up to 3mg after being on 2mg for 10 weeks, 25lbs down. Key for me has been electrolytes every day, high protein intake daily and doing as much walking as possible. Best I have felt in years
4mg Reta 1.5mg Tirz both split dosed twice weekly on alternating days. Down 85lbs since January. Now on Test and tesa/ipa along with Klow. I feel like Iron Man 👊🤣
Is there a difference between small doses (like .35-.45) throughput the week, and one larger dose once a week… if both options total out to the same amount per week?
I have it combined with Tirzepatide. I'm right around 2mg per week and 15mg of Tirz.
I have been on Retatrutide for 12 weeks now. Bumped up this week to 7mg now. I haven’t lost 1 kg 😢 my appetite is down. I’m not hungry and have zero cravings but I still can’t lose any weight.
Taking 1.5 mg/wk and my skin is uncomfortable to the touch, and my RHR is def higher. My lipase levels are high as well.
